Ireland medical registration for Pakistani doctors

Many Pakistani graduates can apply for exemption from the PRES if their house job meets the Irish Medical Council’s equivalence rules. Here is what the current rules actually say.

Can Pakistani graduates skip the PRES?

Yes, many can. The Irish Medical Council treats a Pakistani internship (house job) that commenced on or after 1 January 2009 as equivalent to an Irish internship for the Certificate of Experience route, provided the rotations meet their published standards (including adequate medicine and surgery components).

This is not automatic. You still have to apply for registration, submit the correct documents, and receive a formal determination. Some applications are assessed individually if the rotation details are incomplete.

What you typically need

  • Primary medical qualification and detailed transcripts
  • Certificate of Experience / house-job completion certificate from PMDC showing start date and rotations
  • Evidence that the internship included the required medicine and surgery components
  • English language evidence (IELTS Academic overall 7.0 with no less than 6.5 in any band is the usual minimum; check current rules)
  • Identity documents, good standing certificates, and any higher qualifications if using the Higher Qualification route

FCPS or other recognised higher qualifications can also support exemption under the Higher Qualification route when combined with the required internship or training period.

Practical process notes

Start by reading the current “Registration Rules and Exemptions” and “Certificate of Experience” pages on medicalcouncil.ie. Document lists and fee amounts change. Many applicants open a physiciansapply-style or direct IMC account and upload documents for assessment.

Processing times vary. Incomplete rotation breakdowns are a common reason for delay or refusal of the exemption. Keep the original house-job certificate and any hospital letters that list exact dates and specialties.

After registration

General registration allows unsupervised practice but does not by itself make you a specialist. Specialist training places and further registration have their own rules. Some non-EEA doctors still face practical barriers to training posts even after registration; the regulatory landscape has been changing, so verify the latest position with the Medical Council and the relevant postgraduate training bodies.

Frequently asked questions

Is every Pakistani house job automatically exempt?

No. The internship must have started on or after 1 January 2009 and the rotations must satisfy the Medical Council’s requirements. You still apply and receive a decision.

Do I still need IELTS?

Yes. English language standards apply to almost all applicants. Check the current minimum scores on the Medical Council website.

Is PRES still an option if exemption is refused?

Yes. If the Certificate of Experience or Higher Qualification route is not granted, the PRES route remains available for eligible applicants.
